







| Who We Serve Since 2001, the center has served 35,054 patients with a total of $4.45 Million in "medical value." In 2010, we served 3,827 low-income patients. In 2011, we anticipate providing services to 5,400, an increase due to a full-time Physicians Assistant (PA) being assigned to the Wellness Center at no cost, allowing us to serve more patients. We remain the only medical clinic in this area providing medical urgent care, dental, chiropractic services, plus on-site medical screenings for our patients. In our 10th year of operation, we remain debt-free. Wellness Center patients include residents from both Dallas County (84%) and Tarrant County (16%) and are screened for eligibility based on income and residency. All applicants must be residents of Grand Prairie and be members of a family with family income at or below 125% of the Federal Poverty Guidelines. For dental care only, applicants are accepted who are members of a family with family income at or below 185% of the Federal Poverty Guidelines. We aid families in becoming more independent and productive. A healthier person is more apt to find and maintain a job in the community, with less absenteeism. A healthy child stands a much better chance of learning than one who misses school because of illness. The Wellness Center’s service population experiences higher than the national average rates for the occurrence of cancer, diabetes, heart disease, dental disease, teen pregnancy, late entry into prenatal care, obesity and substance abuse (Texas Council on Alcohol and Drug Abuse, Bureau of Risk Factor Surveillance, and Texas Department of Health). Our History We opened on November 6, 2001 with many civic and church leaders attending—showing broad community support for the project. The need for the facility was realized in 2001, when a mobile health van—previously operated by the Dallas/Fort Worth Medical Center—stopped its weekly visits to a local church due to financial collapse of the D/FW Medical Center. Local residents relied on those free health services which twice saved people in immediate danger of dying. With the help of numerous interested parties, particularly from the pastor and business manager of Immaculate Conception Catholic Church, the Wellness Center facility was built and paid for by donations and grants from community members--including CDBG grants through the City of Grand Prairie. We remain deeply grateful to all those special individuals and organizations who have assisted us in bringing these much-needed services to our community. Funding Our funding comes from a variety of sources including: foundation and family grants, individual donations, as well as CDBG grants and other civic funding. We continue to seek grants from every available source and are always looking to fund our various programs and expand our services and facilities.
